I Owe You a Coffee

The hospital has hired several new hospitalists during the past few months. One in particular really clicked with me. I introduced myself to her because she had several of my patients for the day, who required a lot of attention. I had to call her several times regarding discharge planning/issues, a change in antibiotics, and family concerns.

Typically, many doctors seem to get annoyed by patient/nurse concerns and don’t communicate with the nurses as much as we would like. At the end of the week, this hospitalist repeatedly thanked me for “making her first days easier” and for “telling her any issues/concerns so we can solve them.” She even offered to buy me a coffee sometime for her gratitude.

It is really nice to see true nurse-physician communication without feeling overpowered. She valued my opinion/suggestions, took her time to assess the nurse and patient needs, and treated me like we were on the same team (which we are).

Relationships like this are so important in healthcare, yet so minimal. It is so frustrating to be hesitant to call a doctor for a legitimate concern. It’s frustrating to sense that a physician feels like they have all of the power/intelligence. It is important to remember multi-disciplinary care and recognize that each member is on the same team to serve the patient.

A personal care plan: how to refuel after a shift

Nurses create care plans for patients to guide nursing interventions toward favorable outcomes. We also need to create our own care plans for ourselves in order to continue to care for others. I have created a list of healthy interventions to ensure that I am full, so that I can pour out to others.

  • EXERCISE 3x/week- running, hiking, walking, lifting weights
  • Eat a variety of healthy foods and colors- meal prep for the week to save money and prevent yourself from choosing unhealthy choices
  • Socialize- LIMIT social media. Engage in community events and groups.
  • Go outside- it’s amazing what fresh air can do.
  • Read- fiction, nonfiction, articles, online journals
  • Sleep- Try to take the hour before bed to really wind down from the day.. Stay off the phone and any social media. Read a book or take a nice warm bath.
  • Take a long shower
  • Binge-watch netflix
  • Watch a movie
  • Paint/Color
  • Journal
  • Pray
  • Get your nails done
  • Prepare for the next shift

Tips for new nurses

  1. Nurse the patient, not the computer. New nurses are very task-oriented and focused on completing what needs to be done for the day. It is so easy to focus solely on the screen and not even make eye contact with the human being in the room. It is important to listen to the patient and family, as well as to establish rapport. “If it is not documented, it wasn’t done.” Save documenting until everyone is properly cared for. Do not leave a patient unattended because documenting is behind.. you can always back-time.
  2. You do not work for the doctor, you work for the hospital. This was the most helpful advice my preceptor gave me during orientation. I would get so upset if a doctor yelled at me, but they have no right to do that. We are employed by the hospital, not the physician. Be respectful and follow their orders, but do not be afraid to question orders or ask for clarification, especially if you are concerned about the patient.. They see the patient for roughly 30 minutes, we see them for 12 hours.
  3. Smile. There are many difficulties and frustrations with nursing, but complaining does not solve anything. No one wants to be admitted into the hospital, just smile. It brightens the environment, is proven to make you in a better mood, and people are less likely to yell at you.
  4. Take every opportunity. Learning comes with experience, so take every chance and opportunity to grow. Nursing is a profession, and treat it like one. Be enthusiastic for what you have worked so hard for. Act like you are confident, even when you are not.
  5. Care for yourself. Do something every day that is for you. Whether that being reading, taking a bubble bath, making your favorite food, or watching your favorite show. Make sure you have a healthy you-time to destress from the previous shift. Exercise, eat right, and be kind to yourself.
  6. Set goals. Getting a certification, moving to a new specialty, earning a higher education. Have something to aspire.
  7. Remember you are one person.
  8. Invest in compression socks.
